We provide services throughout the Louisville, KY area under four Kentucky Medicaid Waiver programs — ABI, ABI-LTC, Supports for Community Living (SCL), and the Michelle P. Waiver (MPW) — each outlined below.

Acquired Brain Injury (ABI) & ABI Long-Term Care (ABI-LTC) Waivers

For adults with acquired brain injury, the ABI Waiver supports individuals in active rehabilitation working to reenter the community, while the ABI-LTC Waiver supports those who’ve reached a stable point in recovery and need ongoing maintenance care. We provide:

  • Residential Level 1 & Level 2 (staffed residences)

  • Adult Day Training

  • Occupational Therapy

Supports for Community Living (SCL) Waiver

For individuals with an intellectual or developmental disability, we provide:

  • Residential Level 1 (staffed residence)

  • Residential Level 2 (family home provider)

  • Personal Assistance

  • Community Access

Michelle P. Waiver (MPW)

For individuals with an intellectual or developmental disability receiving home and community-based support, we provide:

  • Community Living Supports

  • Personal Care
